Continuity of care is crucial to receiving the best medical care. We guide you to ensure your physicians are working together, creating one cohesive team working in your best interests and supporting your goals.

In-Home Care

Do you need help in your home, but don't know where to start? Many times, just a little extra help in the home is all it takes to allow you to age in place longer, while ensuring your safety is an upmost priority. When in-home care is needed, we will refer you to only those in-home care agencies which we have vetted and have experience with for proven reliability, top-notch care, and only the best communication.

No matter what your care needs are, finding the best care community is daunting at best. Whether you are in need of independent living, assisted living, skilled nursing care, or memory care, our care coordinators work with you and your family to find that perfect new home for you. Because your care coordinator really gets to know you, our placement recommendations will be based on not only your care needs, but also your social, spiritual, cultural, and financial wants, wishes and goals. When a higher level of care becomes necessary, we will walk with you through this next chapter in your journey as well.

long term care

What is your definition of long-term care (LTC)? We used to define LTC as that of requiring 24-hour nursing care in a traditional "nursing home". We now know that LTC means your long-term care, wherever that care is needed, and whatever level of care is needed. If you have received a diagnosis of a chronic illness, you will need long-term care in one form or another. We will help you with the resources required to get you the care you need, wherever you need it.

medicaid planning & Applications

If you need long-term care, chances are you may need Medicaid or other public benefits. Most people who need Medicaid attempt completing the application on their own, at least once, and are often denied. Once denied, it becomes more difficult to get approval and now you have wasted precious time to get the support you need through Medicaid benefits.

We will monitor your finances with you, over a period of time, all to evaluate if there will be a need for Medicaid, what steps are needed to ensure approval, and the right time to apply. We will then complete your application with you and follow your case through the entire process to approval. Once approved, we will complete your annual redetermination for you, as long as your services continue with Premier Senior Care Coordination.

Are you a senior without any local family? Do you want to avoid being a burden on your family and friends as you age and begin needing care? Your care coordinator will create a plan for you, based on your wants, wishes and goals. When you make those decisions now, you are taking the burden off of your loved ones should there become an issue of incompetency. We have the resources, experience, and knowledge to provide the problem solving and direction you need. We also provide education in the community so we are always going to be providing you with the most current information available

Home health care is typically provided by your insurance when there is a change in your
condition or on discharge from a skilled rehabilitation facility or hospital. There are many
services available through home health depending on your needs at that time. These can include physical, occupational, and/or speech therapy, skilled nursing, medication reminders, Alzheimer’s and dementia care, and much more. Your care coordinator knows what services you may require and how to get these services started.

LTCi is frequently the difference between aging in place and placement. It also is
extremely complicated and time consuming to initiate a successful claim. Your care
coordinator is experienced in getting these claims successfully approved on the first try.
We are also well versed in how to best take advantage of your policy to get the most out
of it.

YES! Having your healthcare documents in place is crucial to ensuring your loved ones
can help you make decisions if you no longer can. PSCC works closely with a local law
firm who can guide you through any legal documents you may need, or which are aging
and need updated.

Yes. PSCC takes your confidentiality very seriously. We will only speak in reference to
your case with persons or entities with which you have given us written permission. Our
client management software system is 100% HIPAA compliant with many layers of

Client advocacy is a top priority for PSCC. We know what good care looks like and will
make sure our clients are receiving it. We know state and federal regulations and will
make sure that you or your loved one’s care team is upholding those regulations and

PSCC will not refer any individual or agency to our clients without getting to know them
very well and vetting their service/product before referring to them. We also get to know
you, our clients, to make sure our referrals are a good fit for you.

PSCC care coordinators are your single point of contact for any and all questions and
concerns relating to your long-term care. We want you to be comfortable calling us even
with what you might believe is a little thing. If you have a fall, let us know. If you are
unhappy with your assisted living community, let us know. If you need a referral to a skilled rehab facility, let us know. If you have questions about your insurance, ask us.
